Neurospine Institute's mission is to offer specialized care for all Neurosurgical conditions. Neurospine Institute has partnered with the University of Florida, Department of Neurosurgery in order to be able to accomplish this goal. Neurospine Institute's West Orlando practice focus is minimally invasive treatment options for cervical, lumbar and carpal tunnel conditions.  If we feel that you have a neurosurgical condition or injury the West Orlando practice does not treat, we will be proud to offer the services of our University of Florida affiliates and coordinate a referral for your convenience. 

Please bring with you the following information and items to your appointment. If the items are not available at the time of your visit, it may result in the rescheduling of your appointment.

• MRI, CT scan report and films. Films must be no older than six months old
• Insurance Card
• Authorization number or referral-if required from health insurance company
• Auto claim information or Signed Letter of Protection from attorney
• All forms listed to be downloaded and completed
